Temporal trend of Acute Chagas Disease in the state of Pará, 2008–2024

Purpose: To analyze the temporal trend in the incidence of acute Chagas disease in the state of Pará from 2008 to 2024, and to assess possible structural changes associated with the year 2020. Methods: An ecological time-series study was conducted using confirmed cases of acute Chagas disease in Pará from 2008 to 2024, obtained from the Notifiable Diseases Information System (SINAN). Annual incidence rates per 100,000 inhabitants were calculated based on population estimates from the Brazilian Institute of Geography and Statistics (IBGE). Trends were analyzed using Prais-Winsten regression, with calculation of the annual percent change (APC). Interrupted time series analysis was also performed, considering 2020 as the intervention point. Results: A significant increasing trend was observed over the entire period (β=0.07741; p<0.001), with an APC of 8.05% (95%CI 4.42–11.80). In the pre-2020 period, the trend was already upward (β=0.10159; p<0.001). In 2020, an abrupt reduction in the level of the series was identified (β=-0.92968; p=0.0047), possibly related to indirect impacts of the COVID-19 pandemic on epidemiological surveillance. In the subsequent period, an acceleration in the trend was observed (β=0.18023; p=0.0308), suggesting intensification of the growth pattern. These findings are consistent with the epidemiological profile of the Amazon region, characterized by the relevance of oral transmission and the persistence of structural determinants associated with the fresh food production chain. Conclusion: Acute Chagas disease showed consistent growth in Pará, with a structural change in 2020 followed by further intensification, reinforcing the need for continuous strengthening of surveillance and control strategies.
