Purpose: To analyze the impacts of physical exercise on individuals with Sickle Cell Anemia. Methods: Systematic review of the literature, the preparation of which followed the recommendations proposed by the Cochrane Collaboration and the Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA). Randomized clinical trials published between 2019–2024 in journals indexed in the WebOfScience and Scopus databases, in English and Portuguese, were included. The articles were evaluated using the Risk of bias in randomised trials (ROB 2) tool to assess the risk of bias using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) instrument, with the aim of measuring the level of evidence of the studies. Results: At the end of the selection, 5 articles were included, with a total population of 77 individuals diagnosed with Sickle Cell Anemia, all of whom were classified in the general assessment of the ROB 2.0 tool as “some suspicions”. However, they were classified as having a “high” level of evidence in the GRADE tool. The articles concluded that physical exercise was able to improve with statistical significance all variables of physical capacity and aerobic response and muscle function, structure and organization presented in the studies. Conclusion: it is possible to conclude that physical exercise improved physical capacity and muscle function in individuals with AF, within the studies included in this systematic review.
